Prices and Office Hours

• Office Hours By Appointment Only.

• Fees to be paid at the time of service
• New Patient by referral only, First Visit $350 – up to 2 hours of physician time
• Follow up visits $200 – up to 90 minutes of physician time.
• TeleHealth or Telephone Consults for current patients: 15 minutes – $125
• House Calls start at $425*

Your Insurance Reimbursement: We are Out of Network for insurance except Medicare PPO for chronic lower back pain. Payment is due at the time of service. We accept cash or credit card (includes FSA/HSA cards). 

A Superbill will be given to you to submit to your insurance or we can submit on your behalf. The amount reimbursed to you will depend on your insurance coverage, out of network out of pocket, deductibles, copayments, fee schedule. 

• This fee is for travel located 5-10 mile radius from office. Available to current patients only.
